New Patient Form

BEGINNING MAY 4TH

We will begin a slow re-opening of our clinic on a daily basis for in-office appointments starting Monday, May 4th. We will continue our screening protocols from April including temperature checks and REQUIRED masks for all patients. Our staff will continue to utilize PPE and promote maximal social distancing. You may be asked to wait in your car after check-in. No visitors unless completely necessary to assist patients. Things may look a little different, but our dedication to your health and vision has never been stronger.
